VA LIAISONS FOR SERVICE MEMBERS SEPARATING/RETIRING FROM THE MILITARY VA Liaisons for Healthcare are there to help you as you prepare to transition out of the military and into the civilian


world.  WHAT IS A VA LIAISON?  VA Liaisons are nurses and social workers who coordinate the transfer of health care from DoD to VA for service members as they exit the military.  VA Liaisons


are located at your Department of Defense (DoD) installation to coordinate your health care needs from the military treatment facility (MTF) to a VA health care facility. If there is not a


VA Liaison at your installation, there are regional VA Liaisons available to virtually coordinate health care access to VA facilities nationwide.  Whether you need inpatient or outpatient


care, VA Liaisons will assist in the transfer of your care at the appropriate time in your recovery and rehabilitation process. VA Liaisons will also: •Collaborate with DoD care managers and


treatment teams regarding VA resources and treatment options available for your care. •Inform you about VA health care eligibility, benefits and services. •Connect you with a Veterans


Benefits Administration (VBA) representative if needed. •Educate and assist you with VA health care registration. •Discuss your individualized goals for your care and ensure your VA health


care team knows your needs and goals. •Facilitate your initial VA appointment being scheduled before you are discharged from the military. •Connect you with the Post-9/11 Military2VA team at


your selected VA facility.                      VA LIAISONS FOR VETERANS TRANSITIONING INTO VA CARE FROM PARTNERSHIP SITES VA Liaisons for Healthcare are there to ensure you have follow up


VA health care after you receive specialized services from a partnership site (i.e., through the Warrior Care Network or Avalon Action Alliance). WHAT IS A VA LIAISON?  Like the VA Liaisons


for service members transitioning out of the military, Public-Private Partnership (P3) VA Liaisons are nurses and social workers who coordinate the transfer of health care from partnership


sites to VA for Veterans as they complete specialized treatment in the community.  Each of the partnership sites has an assigned VA Liaison for Healthcare. Whether you need inpatient or


outpatient care, VA Liaisons will assist in the transfer of your care at the appropriate time in your recovery and rehabilitation process. VA Liaisons will also: •Collaborate with the


treatment teams at the partnership site regarding VA resources and treatment options available for your care. •Inform you about VA health care eligibility, benefits and services. •Connect


you with a Veterans Benefits Administration (VBA) representative if needed. •Meet with you to discuss your eligibility and options for personalized VA care. •Coordinate ongoing VA health


care before you are discharged from the partnership site. •Assist with navigating barriers to VA care & benefits. •Connect you with the Post-9/11 Military2VA team at your selected VA


facility.      Every VA medical center has a specialized Post-9/11 Military2VA (M2VA) team ready to welcome you as you transition from service member to Veteran. Your Post-9/11 M2VA team


will assist you, your family, and caregivers in navigating the VA health care system and support you in achieving your health and wellness goals. Often when service members leave the


military, they find difficulty with sense of identity, financial strain, employment/education barriers, relationship changes, mental/emotional wellness, and physical limitations as a result


of their time in the military. Your Post-9/11 M2VA team is trained in addressing the unique needs of reintegrating service members and Post-9/11 era Veterans and will be there to support


you. Post-9/11 M2VA team members are ready to assist with what matters most to you—from accessing care and resources to developing personalized goals for your care and navigating benefits.


  * Collaborate with DoD and community partners to facilitate your transition to the VA health care system * Educate you on VA eligibility, benefits and services. * Provide transition and


reintegration assistance. * Apply a holistic approach to care planning including integration of Whole Health. Whole Health Home (va.gov) * Facilitate a transition of care in the event of


relocation to another VA facility. * Educate and outreach to DoD and community partners on the VA system and challenges unique to Post-9/11 era Veterans.
