Access through your institution Buy or subscribe Finerenone, a non-steroidal mineralocorticoid-receptor antagonist, has previously been shown in the FIDELIO-DKD trial to reduce the risk of

chronic kidney disease (CKD) progression and cardiovascular (CV) events in patients with predominantly stage 3 or 4 CKD and type 2 diabetes mellitus (T2DM). Investigators in the subsequent

randomized, double-blind FIGARO-DKD trial now report that finerenone therapy is similarly associated with improved CV outcomes compared with placebo in patients with T2DM and stage 2–4 CKD

and moderately elevated albuminuria, or stage 1 or 2 CKD and severely elevated albuminuria. The difference in the primary outcome (a composite of CV death, non-fatal myocardial infarction,

non-fatal stroke and heart failure hospitalization) between the treatment groups was driven mainly by the lower incidence of heart failure hospitalization in the patients treated with

finerenone. Taken together, the two trials show that the CV benefits of finerenone in patients with CKD and T2DM extend across the spectrum of CKD. This is a preview of subscription content,
