Gynecologists guideline). In severely asphyxiated newborns without cord blood gases, postnatal measurements may facilitate therapeutic decisions. OBJECTIVES: To describe BD changes in PA-HIE
during the first 6 hours after birth, correlate values with outcome, and estimate the effect of bicarbonate therapy on BD. METHODS: Retrospective observational study of a cohort of 280 term
infants with PA-HIE defined by modified ACOG criteria (Ekert P et al. J Pediatr 1997). Our admissions to 1 outborn and 2 perinatal centers represent a defined geographic region. Severe
adverse outcome was defined as death or severe neurologic impairment by age 12 m. The time course of postnatal BD was compared graphically between infants with and without severe adverse
outcome. In each outcome group, the BD time course was compared between bicarbonate treated(before the first postnatal blood gas) and untreated infants. RESULTS: Of 280 babies, 117 had
adverse outcome, 78 good outcome and 85 missing data. The graph shows BD with (upper line) and without adverse outcomes (lower line), and normal ranges. Bicarbonate treatment had no effect
